Protect yourself and others. Heart of Ohio Family Health is offering COVID-19 vaccinations and testing to keep our patients safe during this pandemic.
COVID-19 vaccinations are now available at Heart of Ohio Family Health at our Capital Park, Whitehall and Whitehall-Yearling High School locations. We currently offer the Moderna COVID-19 vaccine (a series of two doses administered 28 days apart) and the J&J COVID-19 vaccine (one dose).

What to expect at your COVID-19 vaccination appointment
Please bring your driver’s license or a valid ID and insurance card to your appointment. If you are a new patient, you will be required to fill out our Patient Registration Form. For existing patients, you will fill out the COVID-19 Registration Form for the Ohio Department of Health.
We encourage patients to wear a short sleeve top or loose top for the vaccination. The shot will take less than five minutes. After your vaccination, our providers will have you sit for 15 minutes to ensure you do not have any immediate side effects. Meanwhile, you will be scheduled for the second dose if this is your first dose.

Scheduling Your COVID-19 Testing
To schedule your COVID-19 testing, please call our health center at (614) 235-5555.
If you are a new patient, our call center representative will complete your full patient registration form. Please have your state ID or driver’s license and health insurance information on hand.
What to Expect at Your COVID-19 Appointment
COVID-19 testing is an in-car testing procedure. As we have COVID-19 testing in three locations, please note the different drive-in processes:
There will be a phone number posted outside. Please call that phone number and inform us that you have arrived for your testing. After your phone call, one of our nurses will come to you to ask preliminary questions. Then, one of our doctors will test you.

Yes, wear your mask to help prevent the transmission of COVID-19.
Will my health insurance cover for COVID-19 vaccine and testings?
Heart of Ohio Family Health accepts all Medicare, Medicaid, and most major commercial insurance plans. Additionally, we have a sliding fee scale for those who qualify. No one will be denied care regardless of their ability to pay. For more information on our sliding fee scale, please visit our Patients page.
What vaccine is being offered?
Heart of Ohio Family Health currently offers the Moderna COVID-19 vaccine and J&J vaccine.
When will I get my COVID-19 testing back?
You will get your COVID-19 testing results in 3-7 days.
Do I need to get retested if I’m positive?
It’s not recommended to get retested if you have a positive test result. Follow the instructions that your provider gives regarding when it’s safe to return to work or other activities. If you test positive, we will not retest patients unless it is required by your employer.
