Our clinical pharmacist can work with you and your healthcare provider to develop a personalized medication plan and lab testing to help meet health goals and avoid unwanted side effects. Our pharmacist can assist in managing medications for a variety of health conditions, including:

Medication Review and Education

Additionally, our pharmacist can provide a comprehensive medication review and education on medications. If you would like to have medication education or have medications adjusted to meet specific health goals, we are here to help. Clinical pharmacist services are available to all Heart of Ohio patients who have seen a primary care provider.
