During this time of physical distancing, it is extremely important to continue getting your provider visits. At Heart of Ohio Family Health, we believe in a patient-centered model of care, in all available forms. With our telehealth services, you will be able to see your providers and get your check ups, in the comfort of your own home.

What is telehealth?
Telehealth is the use of video conferences, telephone calls, or images transmitted via email or fax to facilitate clinical health care and education.

How does telehealth operate?
Our patient care representatives will help you make either a phone or video call appointment. Video visits will be completed through Zoom (a free video communications service). Phone appointments may be conducted with any accessible phone.

How do I check my telehealth information?
We recommend you use our patient portal, FollowMyHealth, to check your appointment status, see past medical records, and message your providers.

Does my insurance cover telehealth appointments?
Please check with your insurance provider to see if telehealth is covered by your plan. Heart of Ohio Family Health accepts uninsured patients and offers a sliding fee scale to all who qualify based on income and number of dependents.
