Information for Patients
Heart of Ohio Family Health has two convenient centers to serve the needs of new and current patients: Capital Park Family Health Center and Whitehall Family Health Center. Both health centers currently provide primary medical care, diagnostic laboratory services, referral for dental services, and screening and maintenance for mental health issues. Additionally, the Whitehall Family Health Center provides OB/GYN services.

Sliding Fee Discounts:
Payment policies: We have sliding scale fees for those who qualify. Co-payments are collected at the time of service, with insurance co-pays as shown on your insurance card or a $25 minimum co-payment for those without health insurance. All insurances are accepted.
Spanish interpreting service: Patients may request Spanish interpretation service at Whitehall only. To request services in advance of your appointment, please send an e-mail to Yammah Morgan (firstname.lastname@example.org).
Scheduling your appointment
To schedule your appointment, please call one of our two health centers during office hours to speak with one of our friendly scheduling assistants. If are a new patient or are not sure which center is your medical home, you may call either center for assistance.
